6.  Fill-in Request Time Off form: 

       a.  Date From -  Time off or Leave Date start

       b.  Date To -  Time off or Leave Data end

       c.  Reasons - Type of personal time off e.g.  Vacation Leave, Sick Leave, Paternity/Maternity Leaves, and/or Emergency Leaves

       d.  Cancel? toggle switch - this is use when after filling a leave but later decided to cancel it.  See steps on how to cancel a leave below. 

       

7.   Click Add button to submit.
